Please fill out before your orientation and submit

Youth volunteers age 17 and under must have this form signed by a parent or guardian, granting permission for the youth to volunteer at The Mount Kisco Interfaith Food Pantry.

I understand that the volunteer duties may include any of the following: unloading donated food from cars or trucks and carrying into the Pantry; date-checking, weighing, and shelving donated food items; packing bags of produce; stocking food items in store room; breaking down boxes and carrying to the sidewalk; bagging donated bakery items. I understand that I am responsible for the transportation of my youth to and from The Mount Kisco Interfaith Food Pantry. My youth understands that they are expected to interact respectfully with all staff, volunteers and clients at the Pantry. I understand that if requested, the Pantry may provide a document confirming volunteer community service hours.
